Comment approval and titles

Why do some comments require approval and others do not?
I posted on an old thread that somebody started a long time ago without approval but then when I commented on a thread I started, it said my comment needed approval.
I would PM a moderator or the owner of the forum to ask, but I don't know who those people are or how to find out.
Please don't think I'm not grumpy, I'm just curious and I dislike spam as much as anyone!
Also, please explain what the different titles mean...as in Junior member, Senior member, etc.
