We've all done it, share what you've done and what you now do to prevent it from happening again.
For me, it was when I was backing into an rv park site. I got out of the coach in the rain, and went over the maneuvers needed to back into the site. As I proceeded in reverse diligently paying attention to my back up camera, forgetting about my spotter I did it...I backed into a tree
causing a decent amount of damage.
Today, I use my wife diligently and I have gone a few years without another incident backing into tight places (knock on wood). That is not to say that I don't make goofy mistakes.
